Navigon, Mobile Navigator North America. This is one of my all time favorite apps - it's also the most expensive app I've ever paid for, but I have to say.. it was well worth it. This app easily replaces any and all other mapping apps and GPS units out there. I've owned several GPS units over the years, Magellan, Tom Tom and others, but to-date Navigon is the best GPS application I've had, and its nice to have it built-in to my iPhone so I don't have to carry a separate GPS in the car.
Navigon features turn-by-turn voice instruction which I found to be very fast and accurate. Out of all the GPS navigation systems I've used with voice, this is the only one that gives the instructions well timed in advance of an upcoming turn. It really sucks when a nav system gives you the instruction within feet of the turn or just after the turn causing you to miss it. Navigon keeps you on track and works very well. If you are considering getting a navigation application for your iPhone or iPad, this is my recommendation. For more info check out the Navigon website
